Direct Primary Care

The healthcare landscape is changing daily. We recognize that health care affordability and access is a daily struggle for our community. Therefore, we would like to offer an affordable alternative to access care.

Direct Primary Care (DPC) is an innovative healthcare model in which physicians provide primary care services for a set monthly fee.

What you need to know

• The Direct Primary Care Model is for individuals and families who do not have health insurance or who only have catastrophic health care coverage.
• The Direct Primary Care Model is directly paid by the patient for primary medical care. R Family Medical Group does not accept or bill third-party payers.
• Due to regulatory restrictions, the Direct Primary Care Model is not available for patients who are eligible for or enrolled in Medicare, Medicaid or other government health insurance programs.
• Direct Primary Care is not a comprehensive health insurance coverage nor is it a contract of insurance. It provides only the health care services specifically described in this Agreement.
• You cannot pay the DPC monthly fee with your Health Savings Account (HSA). Please note: currently this issue has been placed before the IRS to change the rules that a DPC fee can be directly taken from an HSA.

The Direct Primary Care Model includes:

• Acute and chronic disease management
• School, sports and workplace physicals
• Mental health treatment for depression, anxiety, and other mood disorders
• Stress Management
• Annual physical  
• Women’s Health (Annual Well-woman exam)
• Unlimited monthly office visits

Fee Information

• A MINIMUM of 3 months of fees is due upon registration with the Direct Primary Care Membership Agreement and this payment is non-refundable.
• Monthly fees are due on the 1st of every month.
